Long Term Community Supports (LTCS) &

Adult Developmental Vocational Program (ADVP)

Both programs’ services provide prevocational, recreational, and social activities within HVO and the community for individuals with intellectual and/or other developmental disabilities.  These activities are designed to prepare the individual to work as independently as possible and provide community inclusion opportunities.  Services are provided in person, on the HVO campus or in the community.  Admissions are authorized by VAYA Health.   

Admission Criteria

The individual must be 16 years or older, have an intellectual and/or developmental disability and not be a danger to themselves or others.

Innovations-Day Supports

Innovation Day Supports emphasizes inclusion and independence with a focus on enabling the individual to attain or maintain their maximum self-sufficiency, increase self-determination and enhance the person’s opportunity to have a meaningful day.  This service is provided with a staff ratio of individual (1:1) or group (1:2 or more).  Services are provided in person, on the HVO campus or in the community.   

Admission Criteria

LTCS and Innovations are a Medicaid service authorized by a Managed Care Organization (MCO).  Referrals can be made by an MCO Care Manager, guardians, other providers and individuals who are interested in this service for themselves. Individuals must receive Innovations Services and have goals written into their Individualized Support Plan for Day Supports.

The Hub (formerly Learning and Enrichment Center)

Services are designed to provide leisure/recreation opportunities, educational classes/activities, and community involvement for individuals who have intellectual and/or other developmental disabilities.  The Hub allows individuals an opportunity to learn and enrich their lives through activities geared to their preferences and abilities. 

 The focus of The Hub is to maintain and/or improve everyone’s physical and mental health by providing elements of a meaningful day.  Individuals who choose to participate may also participate in vocational services provided at HVO and/or in the community.
